Photocopies of the following documents are required:
Divorce Decree (if applicable)
Health Insurance Card
Current Pet Vaccinations (if applicable)
Certificate of Naturalization (if applicable)
Driver’s License
Health Insurance Card
Birth Certificate or Passport
Marriage license (if applicable)
1st page of prior year tax form 1040
You can upload your completed paperwork to your Google Drive folder that we will send you access to. If you need help with Google Drive, click here. Or if you prefer, we are happy to collect your documents when we visit with you in your home.
Our study meets Florida adoption requirements and meets all Florida home study guidelines.
Need a home study update?
Has it been a year since your initial study was completed? If so, you need to update your home study in order to maintain your eligibility.
Traveling outside of Florida for your adoption?
Here is the form needed for the Florida Interstate Compact, should you adopt outside of the state of Florida. This must be completed and given to your placing entity.
