  We will need photocopies of the following documents:

• Driver’s License

• Health Insurance Card

• Birth Certificate or Passport

• First Page of Prior Year Tax Form 1040

• Marriage License (if applicable)

• Divorce Decree (if applicable)

• Current Pet Vaccinations (if applicable)

• Naturalization Certificate (if applicable)

• Military Discharge (if applicable)

You can upload your completed paperwork to your Google Drive folder that we will send you access to. If you need help with Google Drive, click HERE. Or if you prefer, we are happy to collect your documents when we visit with you in your home.
